Office of Registrar

 

 

Welcome to the Registrar’s Office. We are a custodian of SBU’s records.

We serve current students and alumni at SBU. It is our responsibility to:

  • Provide transcripts.
  • Provide enrollment and degree verifications.
  • Complete state licensing forms.
  • Complete name and address changes.

In addition, the Office of the Registrar maintains class lists, oversees class registration, provides graduation information, and provides statistical reporting for the institution.

Whether you call, email or visit, our goal is to provide the necessary help. Our allegiance is to the students, alumni, and faculty of SBU, and we strive to maintain data privacy and to ensure the accuracy, security, and integrity of academic records past and present.

We also ensure that we operate in accordance with the Family Educational Rights and Privacy Act of 1974 (FERPA), protecting individual rights and privacy

Official Transcript

Current, alumni and former students can request a transcript by downloading and printing a document request form. The completed form can be delivered to the Office of Registrar with payment in either one of the following ways:

  • Fax: 714-533-6040
  • Email: whan@southbaylo.edu.
  • Mail: 1126 N. Brookhurst St., Anaheim, CA 92801
  • In-person: Visit the Anaheim campus or Los Angeles campus

Official transcripts are issued by Registrar in five (5) business days of receiving the student’s written request. Any records prior to 1990 may take longer.

Fee:

  • $10.00 for current students
  • $20.00 for all other students

Important Notes:

  • Official transcripts are issued in separate, sealed envelopes.
    • In accordance with the Family Education Rights and Privacy Act of 1974 (FERPA), your records will only be released to a third-party with your written consent.
    • All outstanding university financial obligations must be cleared and missing grades submitted before your transcript can be released.

    Procedure

    The completed form can be delivered to the Office of Registrar with payment in either one of the following ways:

      Change of Address

      Current, alumni, former students should complete the Change of Address Form and submit it to the Office of Registrar.

       

      Change of Name

      Current, alumni, former students should complete the Change of Name Form and submit legal documentation such as marriage certificate, divorce decree or legal change of name document to the Office of Registrar.

       

      Degree Verification

      The degree verifications are available for alumni who have received a degree.

      Degree verifications contain alumnus/alumna name, dates of enrollment and degree received. Degree verifications can be requested by the alumna/alumnus or outside organizations/individuals (HR offices, background check companies, etc.).

      Enrollment Verification

      Requests for enrollment verification, good standing or completion of the program must come from the student, former student or alumnus.

      Verifications may not be taken from a parent or third party unless a release of information form has been signed by the student, former student or alumnus authorizing the release of information to the third party. Requests cannot be taken over the phone.

      All requests for enrollment, good standing or graduation verification must be submitted in writing to the Registrar’s Office.

      The verifications will be faxed or mailed directly to a third party upon request. Any forms that a third party requires must be submitted with your request. Also, include any assigned account or reference numbers so that they may be noted in the verification letter.

      Please allow three to five business days for processing.

      Graduation and Diploma

      Degrees Conferred:

      Degrees for completed programs at SBU are conferred four times a year. Applicants for degree conferral must plan to complete their program requirements by the end of the quarter for which they applied.

      Diplomas:

      Once your academic requirements are completed and your financial obligations to the University have been met, your diploma will be issued in four weeks. It is available for pick up by the student with a photo ID. If you wish to have the diploma mailed, please check for the Diploma mailing fee on DOC form. Diplomas are sent by USPS; the fee is $20 for the United States, $30 for Canada and Mexico, and $50 for mailing abroad.

      Replacement Diplomas:

      In the event that a diploma is lost, stolen or damaged, a request for a replacement, Document Request form, may be sent to the registrar’s office. The cost for a replacement diploma is $100.00, and the process may take from four to six weeks from the time it is ordered. The new diploma will bear a statement in the lower margin, certifying that the diploma is, in substance, reissued.

      Commencement:

      All students who receive degrees in September, December, March, and June of the academic year may participate in the Commencement celebration in May. In April, the Student Service Office sends information about the Commencement Day to all recipients of September, December and March degrees and all applicants for June degrees.
